Violation: Blocked Eyewash
If an employee has an emergency, the eye wash station needs to be easily accessable.
Solution
Keep the area infront and immediately around the eye wash station clear at all times.

Solution
Install the eye shield and instruct personnel to always keep safety equipment in the proper position.

Violation: improper PPE storage
Make sure that PPE is in good condition at all times and properly stored.
Solution
Remind personnel that PPE needs to be returned to designated locations, or disposed of (if appropriate) after use.

Violation: Unmounted Machinery
Grinders must be mounted in two places: to the work surface (either a pedestal or tool bench), which in turn must be secured to the floor (usually with bolts).
Solution
Attach the pedestal to the floor in a manner that is seismically secure, meaning that vibrations from standard operations will not affect the strength of the mount.

Significant risk of injury. Grinders must be securely mounted to the pedistal, which must be securely mounted to the floor.
Violation: Broken Ceiling Tiles

If there is a concern about asbestos exposure, all areas in question should be tested.
Solution
Management should repair all ceiling tiles, and make sure that insulation is properly maintained and not disturbed.

Violation: missing fire extinguisher
Make sure that fire extinguishers are mounted on the wall 3-5 feet above the floor, and that the extinguisher is fully charged.
Solution
Find the extinguisher and return it to the mount. It is a good idea to check the charge if an extinguisher has been moved, because it might have been used.
